Including dementia, chronic medical diagnoses, mental illness, or brain injury.Provides medical, rehabilitation, social services, and therapeutic activities that support adults living at home. Participants attend the Center one to five days per week. Services include nursing care, personal care, social work services, physical and speech therapy as needed. Program offers recreational and social activities. Lunch is served; specialized diets are available.For family caregivers, the program offers: Counseling Case management Respite from the challenges of caregiving
